广州地铁三号线广州东站地处瘦狗岭南麓，地势略呈北高南低，地面高程为13.74～21.13m，北面有广园路，中部为铁路站场，南面为广州东站大片站房建筑。广州东站折返线大部分位于瘦狗岭下，其余部分位于广园东路下方。所处的地形起伏较大，地面高程为19.91 ～105m。广园东路及广园东路以南为山前冲积平原地貌为主，瘦狗岭地段为山地。广园东路上的地面和高架桥上交通繁忙。
广州东站主要包括广州东站及站后折返线暗挖隧道工程。站后折返线暗挖隧道下穿部分瘦狗岭地段、广园东路、广园高架桥等，起点里程为Y（Z）DK0+036.0，终点里程为Y（Z）DK0+336.676，总长为300.676m。广州东站暗挖隧道地表主要为火车东站站区，起点里程为YDK0+336.676,终点里程为Y（Z）DK0+605.182，总长268.51m。车站及折返线隧道防水等级分别为一级和二级。
